IBD Warrior Toolkit

The IBD Warrior Toolkit is a co-created, patient-centered booklet designed to help people living with IBD navigate their treatment journey with confidence. Developed in collaboration with healthcare professionals (HCPs), people with IBD, and AbbVie, the toolkit encourages readers to take an active role in their IBD care—building the knowledge and practical tools they need to better understand their condition, participate in meaningful conversations with their healthcare providers, and advocate for their own health.
Inside, the booklet guides readers through key steps such as:
learning IBD basics (including how IBD differs from IBS and how IBD may extend beyond the gut),
understanding testing and monitoring (blood tests, stool testing like fecal calprotectin, and imaging/endoscopy),
interpreting results as part of a bigger picture rather than single data points,
tracking symptoms and recognizing patterns or triggers,
preparing for appointments and structuring health discussions,
and setting and aligning treatment goals (including remission targets such as symptom reduction and mucosal healing).
Endorsed by IBD organizations and rooted in shared patient and clinical expertise, the toolkit’s overall message is clear: being prepared and informed helps improve communication, supports treatment alignment, and empowers readers to work toward better outcomes in their own IBD journey.
Transition Tools for
Pediatric to Adult IBD Care

In 2024, the Canadian IBD Nurses launched the TransitionIBD program to support young patients with IBD as they prepare to transition to adult care. The program includes resources for patients on their transition journey and resources for healthcare providers supporting them on their transition journey.
The site TransitionIBD.ca guides young patients with IBD through the process of transitioning to adult care. The resource includes a quiz to assess self-management skills and links to other helpful resources to support their disease management.
TransitionIBD FlowSheet

The TransitionIBD Flowsheet Template is based on peer-reviewed evidence and complements the information provided to patients on TransitionIBD.ca. It includes a checklist of actions, a comprehensive medical summary, and discussion topics on the healthcare transition process. The TransitionIBD Flowsheet Template can be added to EMR as a fillable PDF or can be programmed into the EMR as a template by the EMR vendor or healthcare provider.

In 2024, the Canadian IBD Nurses released the latest Vaccination Guide for Immunosuppressed Patients with Inflammatory Bowel Disease. The guide discusses standard Canadian vaccinations, COVID-19 and vaccination in special circumstances.
Early Oral & Topical Anti-Inflammatory Therapies for IBD

In 2025, the Canadian IBD Nurses released the Early Oral & Topical Anti-Inflammatory Therapies for IBD Tool. This tool is a collection of oral and topical therapies used early in the treatment of inflammatory bowel disease (IBD). These early therapies include 5-ASA and corticosteroids. The tool is designed to assist IBD nurses and health care providers in educating patients when discussing treatment options for patients with IBD.

Crohn’s and Colitis Canada is the only national, volunteer-based charity focused on finding the cures for Crohn’s disease and ulcerative colitis and improving the lives of children and adults affected by these diseases.
