Transition Tools for
Pediatric to Adult IBD Care

  • In 2024, the Canadian IBD Nurses launched the TransitionIBD program to support young patients with IBD as they prepare to transition to adult care. The program includes resources for patients on their transition journey and resources for healthcare providers supporting them on their transition journey. 

    The site TransitionIBD.ca guides young patients with IBD through the process of transitioning to adult care. The resource includes a quiz to assess self-management skills and links to other helpful resources to support their disease management.

Advanced Therapies Teaching Tools for Ulcerative Colitis

  • In 2024, the Canadian IBD Nurses released the latest Advanced Therapies Teaching Tools. The tools are a collection of advanced therapies being used in the treatment of Crohn’s Disease and Ulcerative Colitis. They are designed for inflammatory bowel disease (IBD) nurses and health care providers to assist in choosing an appropriate therapy and educating patients when discussing treatment options.

Vaccination Guidelines

  • In 2024, the Canadian IBD Nurses released the latest Vaccination Guide for Immunosuppressed Patients with Inflammatory Bowel Disease. The guide discusses standard Canadian vaccinations, COVID-19 and vaccination in special circumstances.

Early Oral & Topical Anti-Inflammatory Therapies for IBD

  • In 2025, the Canadian IBD Nurses released the Early Oral & Topical Anti-Inflammatory Therapies for IBD Tool. This tool is a collection of oral and topical therapies used early in the treatment of inflammatory bowel disease (IBD). These early therapies include 5-ASA and corticosteroids. The tool is designed to assist IBD nurses and health care providers in educating patients when discussing treatment options for patients with IBD.

Crohn’s and Colitis Canada

Crohn’s and Colitis Canada is the only national, volunteer-based charity focused on finding the cures for Crohn’s disease and ulcerative colitis and improving the lives of children and adults affected by these diseases.

Their site includes information and resources for both patients and providers.

This 2023 report highlights needs of growing numbers and those with more barriers to care.
